Medal struck by the citizens of Forest, Ontario to honour their homecoming veterans. Local communities raised funds to strike medals of this type, or to purchase commemorative gifts to mark soldiers' service. This medal was issued to Lieutenant John William Coultis, who enlisted on 26 August 1915 and was wounded on 9 July 1916.
